Sponsorship Request Form
With your student sponsorship, you will receive a photo and the name of your child within a few weeks after sending your first donation. Periodically you will receive a progress report on the child and age-approriate correspondence from your child. Upon completion of each grade, you will receive another photo of your child.
Linda Glass, Director of Sponsorship, will receive and process your application for sponsorship and initial payment ( check made payable to Haiti Outreach Ministries) and will then assign a student(s) to you. Once the application and payment are processed, you will receive a letter or email along with a picture of your student from one of the coordinators listed here, depending on the grade level of your student. Further communication will be from Jim and Susan Weber for primary students and Bobby and Donna Long for continuing education students. Please do not use the Earthquake donation buttons for Sponsorship payments.
Further sponsorship payments may be made by check, by arranging for automatic withdrawal from your checking account (ACH) or by using the Sponsorship button on the donation page of this website.
